data entry forms
The buttons on the right side of the Visits area allow you to bring up their respective entry forms.

Health Certificate

The Health Certificate form is very simple: Type a destination and departure date. Click Print, and a printable report similar to below will be displayed.

The instructions button allows you to input instructions for care of the animal that you can print and give to your client. If you have a need to frequently issue the same type of instructions, for instance after spaying or neutering, you can save your instructions as a template, and use them over and over again. When you use the instructions button, the instructions given to the client are retained in the visit record for future reference.

The Vaccination Certificate form is also very simple: For rabies vaccinations, select Tagseries (rabies tag number for 1 or 3 year certificates), select the vaccine type, and the other relevant fields will be filled in automatically. Click Print, and a printable report similar to the one displayed below will be displayed.
You can click the buttons alongside the other vaccinations to indicate the vaccination(s) given. This will cause the current date to be inserted for the specified vaccination(s).

The Bloodwork form is more complex. There are some fields that are calculated, and will be filled in automatically. There are 4 places for photos to be included ... click on an empty photo, and you will be presented with a file selection dialog, where you can select the photo of your choice. If there is already a photo present, clicking on the photo will display it in a larger view. When you have entered the information you need, you can click Print, and a printable report similar to below will be displayed.

The remaining buttons all produce similar forms/reports, with the exception of the Photos button, which simply provides a place to put up to 8 photos of interest that are not necessarily related to any of the other input forms.
It is important to remember that all of these entry forms (as well as the printed reports that accompany them) are ALWAYS related to the current patient and visit record as displayed in the patient visits form.
